Return Material Authorization Request Form

1) Please download and fill out the Repair Form by clicking on the link below.

2) When completed, submit your request to [email protected], and we will respond with an RMA Number to included with your shipment. This number is used to process your order in a timely manner and for tracking purposes.

3) Please be sure to include the RMA Number/Form with your inbound shipment and keep a copy for your own records.

4) Please follow the safety and shipping directions given below and provide as much detail as possible regarding the problem or issue you are experiencing.

• If you have an administrator name and/or password for your device(s), please include for troubleshooting and quality testing purposes.
• Please over-box your products/units in shipping and make sure they are properly secured and have the required packaging materials.
